Registration Form The registration form is a form we use to collect information about a volunteer which enables us to record their volunteer hours.  It allows us to have information to contact the volunteer and determine which program they will fit into, RSVP (Retired Senior Volunteer Program) which is for volunteers 55 and older or the Volunteer Center which is for volunteers 54 and younger.  The information given to us is not shared or sold and is held confidential. click here to download form

Daily Time Sheet The daily time sheet is used to report hours on a daily basis, instead of monthly.  This form allows the volunteer to show what they did on a specific day for a specific amount of time.  It also has a column in which volunteers 55 and older can request mileage reimbursement.  We will reimburse mileage from $5-$50 per month.  Please remember to get a station manager signature before turning this form into our office. click here to download form

Summary Time Sheet The summary form is used to report hours in a more general way.  You can report an amount of hours spent on a project in a month as a whole instead of each individual day.  This method is quicker and easier to use if you are not requesting mileage reimbursement. click here to download form
